This registration form will record the first 36 registrations. It will continue to accept registrations past the 36 participant limit, but the participant will be notified that they are on a waiting list. The participant must be pre-registered using this form to attend the clinic. Payment for the clinic event must be received by June 1, 2012, or the participants pre-registration will be deleted and the clinic reservation given to the next participant on the stand-by list.
The Hold Harmless Minor Release will be signed by the legal guardian on July 8, 2012 during event check in.
If necessary, the Guest Membership application will be signed by the legal guardian on July 8, 2012 during event check in.

The FIRST10 PAID Pre-Registered Tournament or Clinic Participant will receive a remote controlled mini helicopter in their goodie bag during registration.
